A premature ventricular contraction occurs when a focus in the ventricle generates an action potential before the next scheduled sinoatrial nodal action potential. PVC’s occur when an early electrical impulse occurs from a location in either ventricle. While patients may be asymptomatic, typically these PVCs cause sensations of skipping, heart pounding, and possibly chest pain, shortness of breath or dizziness. Doctors frequently ask for a rhythm strip (a prolonged EKG recording) to be performed at the same time as an EKG to increase the chances of detecting premature ventricular contractions and other abnormal rhythms.


PVC’s are extra beats which occur from an ectopic focus on the ventricle wall. In the normal person these may be caused by smoking, alcohol, or coffee ingestion. They usually are rare and inconsequential in normal persons. However, very frequent PVCs can be symptomatic of an underlying heart condition (such as arrhythmogenic right ventricular cardiomyopathy ). Often the retrograde P wave can be seen on the ECG , hiding in the ST-T wave of the PVC. Another type of ECG is called an exercise stress test.


This is illustrated below. If exercise seems to cause extra beats, you may be at higher risk of other heart rhythm problems. They may occur in individuals with healthy or unhealthy hearts. PVCs are fairly common and increase in frequency with aging. Normal causes include anxiety and emotional stress (increased sympathetic stimulation) as well as after consumption of caffeine, alcohol, or nicotine.
